Betsy Weiss is inviting you to a scheduled Zoom meeting.
What is Advanced Care Planning?

Advance Care Planning will center on developing or reworking a document that chooses a medical decision maker for you and communicates to that person your wishes about your medical care and end of life wishes if you are unable to communicate them yourself.

Betsy will offer timely information, define ACP, assist participants to set plan to get documents completed. Let’s discuss this moment and why ACP is important.
